Coverage provided by LifeShield National Insurance Company is through a certificate of insurance issued under a group life insurance policy as part of your membership in the Wellness Association of America. While these certificates of insurance are guaranteed to be issued, the coverage has a graded death benefit amount that is less than the face value of the death benefit in policy years 1 and 2. Member’s coverage subject to policy terms, conditions, limitations, exclusions and state availability. Membership options include 5-year renewable and convertible Term Life Insurance.
